The application for a residence permit and the application for its extension must be submitted on the official application form. Otherwise, it is inadmissible and the procedure will not be initiated.1
The forms must always be filled in solely in the Czech language.

Common Forms
- Application form of reporting changes – the form is used for foreign national‘s obligation to report changes (most often change of address, travel document or name)
- Application form for a certificate of current residence

- The application form for change of employer, work position, employment on another work position with the same, or a different employer:
- If the employee cardholder’s notification of a change of employer, job classification or employment to another position in the same or another job is not made on an official form, it shall be deemed not to have been submitted.
The application form can be completed and sent electronically. Further, a Sample Employer‘s Statement of the foreigner’s professional competence for the required job is attached.

There is a possibility of submitting an Employer´s Statement of the foreigner‘s professional competence for the job in case of a change of employer, position or job for a different work position with a current or a different employer only. However, it is not possible to submit this statement in case of applying for an Employee Card, or for an Employee Card validity extension. In cases, where documents proving professional competence are required, either originals or certified copies of these documents have to be submitted.
